New Patients

• Available for new patients only.
• Our new patient offer includes:
  i. 1x comprehensive dental examination
  ii. 1x scale, clean and polish
  iii. Panoramic x-rays (if required, the appointment increases from $199 to $260 per person)
  iv. 1x fluoride treatment

• For non-health fund members or for new patients who may choose not to utilise their private health insurance for any reason, this offer is available for (insert relevant price here) (GST-inclusive)
• For health fund members:

i. if St Mark Dental is a preferred provider for your health fund and you have adequate dental cover , the check-up and clean will be provided to you at ‘no gap’, i.e. you will not need to pay any fees ‘out of pocket’ and you may be able to access additional exclusive benefits. We are preferred providers for a large number of health funds, please check eligibility with your health fund or (link for practice preferred providers) for more information.

ii. if the St Mark Dental is not a preferred provider for your health fund, then regardless of your dental cover, you may have to pay ‘out of pocket’ costs to redeem

• Regardless of whether St Mark Dental is a preferred provider for your health fund, we recommend that you check your health fund policy to determine which portion of your fee is covered. While St Mark Dental accept all major health funds, not all health funds offer the same level of benefits, and your level of cover will affect your “out-of-pocket” costs.
• Private health funds determine the rebate amount, if any, that they will provide to us for the services we provide. We will claim the full benefit amount from the health fund for the services included in this offer.
• Optional OPG x-rays will be charged as an additional cost.
• To ensure you receive the most appropriate treatment, your dentist will evaluate your teeth condition at your appointment to consider whether the treatment included in this offer is suitable for you. If we consider it is not, i.e. you may need alternative/additional treatments, we will offer details of those treatments to you, including details of our standard rates. We will otherwise provide the treatment included in this offer to you at a later agreed date, having regard to your teeth condition.

Except for the ‘Refer A Friend’ Program (full terms and conditions, including non-participating practices, is accessible [insert hyperlink here], this offer cannot be used in conjunction with any other offer(s).

Families

Book your family’s dental check-up and clean at St Mark Dental. Health fund members with adequate dental cover can receive gap-free treatment, or $199 per person for non-health fund members (Consultation). During the Consultation, our dental professionals will:

1. conduct a comprehensive dental examination;
2. conduct a standard scale, clean, and polish;
3. provide fluoride treatment; and
4. if reasonably required, take Panoramic (OPG) x-rays for an additional $61 per person (total appointment cost is $260 per person).

Family Booking Requirements

• The family member booking the Offer must book all of the appointments at the same time.
• You must book your consultation in advance. Consultations are subject to availability.

Offer Period

Redemption Process

To claim this Offer, you must book your appointment and mention this offer before your appointment or at reception.

Offer Restrictions

The following restrictions apply to the eligibility and use of the Offer: only one Offer can be redeemed per family of 4 (2 Parents + 2 Children); this offer is only available at St Mark Dental; the person redeeming the Offer must be 18 years or older; and this Offer cannot be used in conjunction with any other offers

Terms and Conditions

1. You will be required to fill out a medical history form when you attend St Mark Dental (and prior to your Consultation) to assist St Mark Dental’s dentist evaluation. There is no obligation for you to undertake additional treatments following the Consultation.
2. If you have private health insurance, it is your responsibility to check with your health fund which portion, if any, of your fee is covered to determine if you are eligible for gap-free treatment.
3. If the dentist determines you need Panoramic (OPG) X-rays, this will be an additional $61 per person, bringing the total to $260 per person.
4. This Offer cannot be used in conjunction with any other offers, subject to our ‘Refer a Friend’ offer.
